*   ALL service requests must be in writing (FAX or EMAIL) and shall be made at least 3(Three) business days in advance to ensure availability. Sunday's, Holiday's and Early/Late hours rates are available by appointment only upon SPECIAL request.

*Most Insurance Carriers require Prior Authorization (PA) for medical transportation services to be obtained. Please contact your Carrier in advance for an authorization prior to your inquiry for services. In order for services being rendered, please have your Carrier FAX written Prior Authorization promptly directly to us at  (732) 613-2626 

 

If your insurance carrier is not listed above, payment is due at the time of services rendered. Patient/Customer shall be responsible for filing transportation claim/invoice with his/her insurance carrier for reimbursement.
